CMS Skilled Nursing Facility (SNF) QRP Report

This report contains information on the three cross-setting Quality Reporting Program (QRP) quality measures:

• Cross-Setting Falls with Major Injury Measure: Application of Percent of Residents Experiencing One or More Falls with Major Injury (Long Stay)

• Cross-Setting Pressure Ulcer Measure: Percent of Patients or Residents with Pressure Ulcers that are New or Worsened

• Cross-Setting Function Quality Measure Application of Percent of Long Term Care Hospital Patients with an Admission and Discharge Functional Assessment and a Care Plan that Addresses Function

When running the report, select a Report Date and a Comparison Date to compare two timeframes of data. Information displayed will be for a full 12-month period prior to the selected dates. Measures will be calculated according to the CMS QRP Manual for each QRP, for that timeframe.

Once you run the report, the report will default to the detail view. You can collapse the report by checking the summary box.

Detail view

The constant, coefficient, and national mean values are those that are provided by CMS. If the current CMS values for a quarter are unavailable, the report will carry forward the CMS constants from the prior quarter until values for that quarter become available in MDS Intelligence. If there’s no data for the prior quarter to carry forward, you’ll see “no data available.”
